台灣由於高齡化進展快速，長期照護需求量也快速攀升。政府自2003年推動「照顧服務員」訓練課程以來，全台已訓練照顧服務員11萬人，惟2014年衛福部照護人力資源盤點結果，僅2萬6千人投入職場。
照顧服務員工作除錢少、事多、重度勞動外，因入行門檻低，不受社會善待與尊重(頂多被視為「半專業」人力)，照顧服務員很難從工作中找到自我價值，且不管工作10年或8年，工作內容始終千遍一律，既無升遷管道，亦缺乏職業魅力。
遺憾的是，長久以來，照顧服務人力招募不易、流失率高、薪資待遇低、職業地位無法提升…等問題明確，產官學各界也一致認為亟需合理的職涯規劃、更精進的專業素養，以提升他們的工作價值觀，但卻遲遲不見藥方出爐。
日本同樣有人力不足，離職率高的擔憂。為促進照護人力終身留任在照護職場，日本於2013年4月完成簡單易懂的「照護人才職涯路徑」，核定不同職等不同薪資制度。
簡化原本複雜多元的「照護職員基礎研修(500小時)」、「居家照護員1級研修(230小時)」，「居家照護員2級研修 (130小時，2013年廢止)」，並且一體化創設「照護職員初任者研修 (130小時)」基礎課程。此課程若加上「實務者研修課程(230小時)」即可順勢與國家考試資格的「照護福祉士」課程銜接，以便鼓勵所有照護工作者未來都朝向國家資格級的「照護福祉士」水準提升。
基礎課程特色除強化「心理與身體構造與生活支援技術」等照護理論技術一體化實施外，同時增加多職種合作的團隊照護時數， 以順利推動社區整體體照護體制、增加「照護與醫療結合」時數、新增「理解失智症」相關科目，以因應失智症老人增加時的照護需要。
